The clinical social work team at Rhode Island Hospital provides care to patients with complex and diverse needs, offering a range of services from assessment, mental health counseling, support, education and more. Social workers and case managers have an excellent understanding of the typical treatments and course of care for individual patients with complicated illnesses and are well equipped to help families cope. The social work and case management teams play a vital role as advocates for patient centered care. They establish relationships with patients and family members, as well as with other members of the patient’s care team, for total patient and family-centered care. |
