Because of the confidential nature of the records, we do not routinely fax medical records, although you may fax us your request. If you would like copies of your medical record, please call the medical records department (Health Information Services). You may pick up the copies in person with a valid driver's license or other picture ID, or copies will be mailed to you. There may be a charge for copies of the record.
