Patient Rights and Responsibilities
In order to provide you with the best possible care, we need your
cooperation. Please tell us about previous illnesses, hospitalizations,
medications, allergies and other matters relating to you health
history.
You will receive a questionnaire from the anesthesia department
prior to admission. If you are scheduled for a pre-admission appointment,
please bring the completed questionnaire with you. If you don't
require a pre-admission appointment, please bring the completed
questionnaire with you the day of surgery.
It is also important for you to know which, if any, of your usual
medications you should take before surgery. If you have not been
provided with this information, or the time to arrive at the hospital,
call you doctor or the Ambulatory Surgical Center at 401-444-8372.
